Experiencing an Issue with taking the WOTC Survey?

There are two options to take the survey.

If you are still experiencing an issue, please fill out the information on the form to complete the WOTC survey.

Fill out this form:

    9. I authorize all third parties to provide to the Company any personal information necessary to determine my eligibility for federal or state tax credit programs. This includes, but is not limited to, information regarding my driver records, military service, SSI benefits, vocational rehabilitation services, unemployment benefits, AFDC/TANF benefits, Food Stamp benefits, and criminal history. I further authorize the Company to complete any forms required to document this information on my behalf, including SSA Form 3288 and IRS Form 8850.  Under penalties of perjury, I declare that I gave the information above to the Company on or before the day I was assigned a job, and it is, to the best of my knowledge, true, correct, and complete.
